Description of intervention(s) / exposure
This study will observe patients who have been discharged from Cabrini's Emergency Department and are eligible for a nurse telephone follow-up call service (which occurs the day after discharge as part of standard care at Cabrini). Patients who have re-attended the emergency departments prior the call or already have a Emergency Department medical review are not eligible. Telephone calls are estimated to take 1 to 20 minutes based on patient needs and interventions required (e.g. medication/clinical advise, facilitating follow-up, patient recall).
